These Terms and Conditions govern the access and use of the website, services, and monthly subscription plans offered by Pet Health Plans LLC, located at 2320 W Flagler St, Miami, FL 33135, United States.

By subscribing to or using our services, you acknowledge that you have read, understood, and agree to be bound by these Terms.

SCOPE OF SERVICE

Pet Health Plans provides subscription plans that grant preferential access and exclusive discounts on emergency veterinary services through affiliated clinics.
We are not an insurance company and do not directly provide veterinary services.

ELIGIBILITY

Only individuals who are at least 18 years old and have the legal capacity to enter into contracts may subscribe. Our services are limited to the State of Florida and are intended exclusively for dog and cat owners residing in this jurisdiction.

PLANS AND SUBSCRIPTION

Our plans are billed monthly and renew automatically every 30 days from the date of enrollment. By subscribing:

• • You authorize recurring charges to your selected payment method.
  • The monthly rate is fixed but subject to change with at least 15 days’ prior notice.
  • You may cancel at any time before your next billing date.

No refunds will be issued for billing periods that have already started, except as noted in Section 8.

COVERAGE DETAILS

Each plan provides pre-established benefits, including:

• • Waived emergency consultation fees
  • Tiered discounts (10%-40%) on veterinary procedures, based on the selected plan
  • Immediate coverage upon successful subscription

Veterinary care is provided solely by independent clinics. Pet Health Plans does not guarantee availability in all geographic areas or the quality/outcome of the medical care received.

LIMITATION OF LIABILITY

PHP is not responsible for damages, losses, or expenses resulting from the services provided by third parties, including affiliated clinics. Our liability is limited to facilitating access to the benefits outlined in your plan.

CHANGES AND MODIFICATIONS

We reserve the right to modify these Terms, subscription prices, and benefits at any time. Any such changes will be communicated via email and posted on our website. Continued use of the service after notice of changes constitutes acceptance.

CANCELLATION AND TERMINATION

You may cancel your plan at any time through your account or by contacting customer support. Cancellation will take effect at the end of the current billing cycle.
Pet Health Plans may suspend or terminate your access if misuse, fraud, or violations of these Terms are detected.

REFUND POLICY

No refunds will be issued for unused services or cancellations made after a billing cycle has started, except in the following circumstances:

• • Death of the pet (official documentation required)
  • Closure of affiliated clinics with no reasonable alternative
  • Subscriber’s legal or medical incapacity

In such cases, Pet Health Plans may issue a partial refund at its sole discretion.

PERSONAL DATA PROCESSING

Pet Health Plans collects and processes personal data in accordance with its Privacy Policy and in compliance with applicable federal and state laws, including the California Consumer Privacy Act (CCPA) and the Florida Information Protection Act (FIPA).
We do not sell your personal data to third parties. You may exercise your rights to access, correct, or delete your data by contacting us at: privacy@pethealthplans.com

INTELLECTUAL PROPERTY

All content on this website, including logos, text, and trademarks, is the exclusive property of Pet Health Plans or its licensors. Unauthorized use without written consent is strictly prohibited.

FORCE MAJEURE

Pet Health Plans shall not be held liable for service interruptions due to events beyond our control, such as natural disasters, internet outages, labor disputes, or public health emergencies.

GOVERNING LAW AND JURISDICTION

These Terms shall be governed by the laws of the State of Florida. Any disputes shall be resolved exclusively in the courts of Miami-Dade County, Florida.
